What the research shows
The classic Baker-Gordon peel left the skin chalk white. Lower-strength formulas have largely replaced it, and they tighten skin and improve wrinkles with a subtler result (Wambier 2019). Gregory Hetter showed that croton oil, more than phenol, sets how deep the peel goes; phenol alone barely peeled the skin (Hetter 2000a). Weaker croton oil solutions healed in about a week and stronger ones in 9 or 10 days, and the strongest almost always caused loss of pigment; Hetter mapped a different strength to each zone of the face (Hetter 2000b).
The result lasts. In women who had a deep peel and a facelift years later, skin taken at the facelift showed a new band of collagen and fine elastic fibers under the surface up to 20 years after the peel. Pigment cells remained but made less pigment, which explains the lighter skin (Kligman 1985).
On the upper lip, a phenol peel smoothed lines more than a CO2 laser on the other side of the same lip (Chew 1999). In a split-face comparison, peel and laser were equal on thin skin, and the laser did better on the thick, oily skin of the chin and nasolabial fold at the cost of more lightening, discomfort and redness (Langsdon 2000).
The peel pairs well with a facelift. A perioral peel at the time of a facelift reduced lip lines and made patients look younger to outside raters (Ozturk 2013), and it shortened the upper lip (Janssen 2026). A phenol peel should not go on skin that has been lifted off the tissue beneath it, because it can cut off the blood supply; TCA up to 40% can be used there (Gonser 2017). A one-stage lift, fat grafting and peel had no more complications than each step alone, though phenol left permanently lighter skin in about 1 in 18 (Kaye 2016).
Phenol passes through the skin into the blood. During fully monitored full-face peels, about 1 in 15 had an irregular heartbeat, more often those with diabetes, high blood pressure or depression, and it was controlled with lidocaine when needed (Landau 2007). Kidney failure and fatal poisoning have been reported, so the peel belongs with trained physicians and constant monitoring (Marçon 2025). Surgeons use different depths on different parts of the face and neck to balance safety and effect (Sterling 2020).
Where surgeons disagree
- What makes the peel work. Hetter's work points to croton oil (Hetter 2000a), and an animal study traced the new collagen to compounds in croton oil called phorbol esters (Justo 2021). A blinded biopsy study found phenol without croton oil gave the same long-term result and named the way the peel is rubbed in as the larger factor (Stone 2001).
- Peels in Asian skin. A modified phenol peel improved wrinkles in Asian patients, but temporary dark patches followed in about 3 in 4 (Park 2007). Experience from the International Peeling Society supports deep peels for dark circles under the eyes in skin of color, with improvement lasting a median of two years (Soon 2023).
What is new
- Frontal fibrosing alopecia, a scarring hair loss at the hairline, is now considered a firm reason not to have a deep peel after patients with it healed poorly (Landau 2024).
- Delivering the peel with a tattoo machine reached deeper and made more new collagen than painting it on, in pigs (Ghanian 2026).
- The perioral peel at facelift shortened the upper lip, a newly measured benefit (Janssen 2026).
What is still unknown
- How tattoo-machine delivery and other new methods work in people. The studies so far are in animals (Ghanian 2026).
- Which ingredient and which technique matter most. The answer still rests on small biopsy studies (Stone 2001, Hetter 2000a).
